After everything I've been through, there's one thing I still struggle to understand: why do many clinics offer meibomian gland expression only as part of an IPL treatment package?
To me, that simply doesn't make sense. IPL and gland expression are two different procedures. They have different purposes, and although combining them can be beneficial for some patients, that doesn't mean everyone needs both.
I believe there are patients with thick, poorly flowing meibum who have no obvious signs of significant inflammation and who might benefit from professional gland expression without necessarily undergoing IPL. That's actually me.
Of course, the underlying cause of their MGD still needs to be investigated, and expression won't be appropriate or sufficient for everyone. But shouldn't the option at least be considered?
My own experience made me question this approach even more. After I stopped IPL, I continued having gentle gland expression for a long time. And over multiple appointments, I saw the secretion gradually change. I can't say exactly what caused that improvement, but I know that expression remained part of my treatment long after IPL was no longer an option for me.
And there's also the question of cost. If a patient doesn't need IPL, why should they have to pay for it just to receive gland expression? A standalone expression appointment should reasonably cost less than a combined IPL and expression session, even though the exact price will depend on the clinic, equipment, and time involved.
I'm not arguing that IPL and expression shouldn't be combined. Sometimes that combination makes perfect sense, and research suggests it can be more effective for certain patients.
What I disagree with is treating the combination as the ONLY option.
Patients deserve to understand what each procedure does, why it's being recommended, and whether a less expensive, less intensive alternative might be appropriate for their particular situation. Because treating MGD shouldn't be about choosing a package. It should be about choosing the treatment that actually makes sense for the patient.
